Add checkbox in a grid (while creating a form)

While creating my form, I need to add a checkbox in my grid.
How can I do this feature ?

    Hi.
    How about you give it a go with Spreadsheet.
    I suppose you can achieve your use case by using this. as it has the checkbox option in it.
